Our communities have a huge role to play in the successful delivery of palliative care, support for bereaved friends or family members or just in initiating important conversations around advance care planning and death.
If you, your community or your organisation are doing work in this area, we’d love you to share it with us and our conference delegates.
Palliative care and aged care are intrinsically linked.
There are many aged care providers, both residential facilities and in the community, who are doing fantastic work in providing high quality palliative and end of life care to Tasmanians with a life-limiting illness.
If this is you or your organisation, we invite you to share your work with us and our conference delegates.
In Tasmania, we are fortunate to have private and public hospitals, dedicated palliative care units, GPs, Multipurpose Centres and a number of other health providers and professionals delivering high quality palliative care across the State.
We invite you to share your stories, work, experiences and knowledge with your clinical colleagues and interested community members.
